Privacy statement

This statement explains how PDC Rx handles the information you submit through our free pharmacy cost analysis form.
What we collect
Your name, hospice name, email address and phone number; your current pharmacy pricing structure; any per diem figures you choose to enter; the invoice and claims files you upload; and your comments. To protect the form from abuse, we also record your IP address and browser type.
Please do not send patient information
We do not need or request protected health information (PHI) to prepare a cost analysis. Before uploading, please remove patient identifiers such as names, dates of birth, medical record or member numbers, and addresses. Drug, quantity, cost, date and pharmacy details are all we need.
If identifiable patient information is included anyway, we will protect it with the same safeguards described below, use it only for your cost analysis, and delete what is not needed. If your organization prefers to share identifiable data, contact us first so we can put a business associate agreement (BAA) in place.
Your agreements with other providers
Your current pharmacy or PBM agreement may limit who you can share pricing or invoices with. By submitting, you confirm that you are permitted to share this information with PDC Rx for this purpose.
How we use your information
To prepare a pharmacy cost analysis for your hospice and to contact you about it and about PDC Rx services. If you no longer want to hear from us, email CostAnalysis@pdcrx.com and we will stop.
Who we share it with
We do not sell your information. We share it only with service providers that host and protect this service on our behalf (Microsoft, for secure hosting and storage, and Cloudflare, for the security check on this form), under confidentiality and security obligations.
How we protect it
Information is encrypted in transit and at rest. Every uploaded file is scanned for malware and held in a restricted area until our team verifies your request. Access is limited to the PDC Rx staff who handle cost analyses.
How long we keep it
Files that are not used for an analysis are deleted automatically within 30 days. Files used for an analysis, and your contact details, are kept for 12 months after the analysis is completed, unless you become a PDC Rx client, and are then deleted. You can ask us to delete your information sooner at any time.
Cookies
This form uses only essential cookies: a security token that protects your submission, and Cloudflare's security check. We do not use tracking or advertising cookies.
About the analysis
The cost analysis is an estimate based on the information you provide. It is not an offer, quote or contract, and there is no obligation to use PDC Rx services.
